He had an AC sprain, then reinjured it. AC sprains are not at all uncommon in football, even if they are more concerning for a QB's throwing shoulder. But he had surgery done by arguably the most well-known orthopedic surgeon in the country who I'm sure was able to articulate for every team exactly what the procedure was, timeline for complete recovery, and long-term prognosis. When Hardy first got shot, I'm not sure anyone knew for certain whether he would play again period, let alone to what extent his recovery would be completely back to his original strength/speed.
